By enrolling in a class, workshop, training program, or scheduling a physical therapy or private fitness training appointment, you agree that you have read, understand, and agree to all terms, conditions, and policies.

Privacy

Body Technic Systems®, Inc. retains the contact information of its clients. It is Body Technic Systems®, Inc. policy not to sell or distribute the contact information we obtain. From time to time, Body Technic Systems®, Inc. may send information about upcoming events and classes to people in our database. To be added to or removed from our distribution lists (email or land mail), please email info@bodytechnic.com or call.

Payment

Physical Therapy and Private Fitness Training

Twenty-four (24) hours notice is required for cancellation of an appointment of any type (Admissions Interview, Physical Therapy Appointment, Private Fitness Training Appointment). Clients who cancel appointments with less than 24 hours notice will be charged for the appointment time. The minimum fee for missed appointments is $60.00.

By Ohio state law, physical therapy clients must obtain written physician’s referral or order prior to arriving at the initial appointment. Lack of physician’s referral is not an excuse for canceling an appointment with fewer than 24 hours notice. It is the client’s responsibility to check on the status of the referral/order and to cancel, if necessary, with 24 hours notice.

Private Training cards expire 3 months (5 hr. package) and 6 months (10 hr. package) from their issue date.
Body Technic Systems®, Inc. makes no guarantee of reimbursement for physical therapy services. Clients should call their insurance companies to verify eligibility and reimbursement rates.

Body Technic Systems®, Inc. does not accept assignment of benefits and is not a member of any insurance provider networks.
Body Technic Systems®, Inc. is not a provider for Medicare or Workman’s Compensation.

Classes

Body Technic Systems®, Inc. reserves the right to cancel a class at any time for any reason without further notice to the public. Registered participants in the class prior to the start date will be notified. If you are not registered for a class, please call prior to dropping in.
Clients may be denied the right to drop-in to a class at the discretion of the instructor for any reason, including maximum class capacity or a determination that a client’s safety may be compromised by class participation.

All missed classes (in the six-week group class sessions) must be made up within the six-week session or forfeited. Body Technic Systems®, Inc. provides ample opportunity for students to make up missed classes. Ask your instructor or call the office for details.
Shoes must be removed prior to entering the studio. Students may wear socks unless otherwise indicated. Space is provided for shoes and coats.

For your safety, please do not chew gum or candy in class. Additionally, no food or beverage are permitted in the studio. Water is provided outside the studio. Please remember to turn off all cell phones and pagers prior to class. Medical professionals and clients with possible emergency situations may leave phones and pagers on vibrate mode.

For the comfort of all our clients, please do not wear heavily scented colognes, perfumes, shampoos, lotions, or other body products during exercise or physical therapy at Body Technic Systems®, Inc. Please do not wear nail polish or other products which may rub off on floors or mats during exercise at Body Technic Systems®, Inc.

Teacher Training - BTS Pilates and GYROTONIC®

All fees for training must be paid prior to the first day of class based on published fees at the time of enrollment.
Printed materials used during training may not be used or reprinted in any manner without written permission of Body Technic Systems®, Inc.

The trainee agrees s/he will not open a studio or clinic within a 10 mile radius of Body Technic Systems®, Inc. without written permission from Body Technic Systems®, Inc. Arrangements for missed training are the responsibility of the trainee and may incur additional fees.

Release and Waiver

Body Technic Systems®, Inc. assumes no liability for any injury that occurs during or as a result of the training, or due to prior conditions.
All participants of physical therapy, private fitness training, teacher training, and group classes are required to sign the BTS Release and Waiver prior to. Print out, fill in and mail or fax this form to Body Technic Systems, Inc.
