Most insurance companies will require that you get a physician’s referral for physical therapy, which you need to bring on your first appointment. In Ohio you are NOT required to come to us with a physicians referral. If you do, in most cases it is very helpful to have a physicians referral and aids with the reimbursement process. Medicare requires the patient to be seen every 30 days by their physician to continue with therapy.

The initial visit takes a little longer than the subsequent visits due to the paperwork that you need to fill out, in addition to the completion of the assessment process. Normally, appointments are usually between 60 and 90 minutes.

You will need to attend physical therapy sessions until you and your therapist decide that you have reached your desired level of function. Often therapy will also be monitored by your primary care physician, or other adjunct practitioners.

Have your physician of record fax to our office the Bureau’s c-9 form to 440-248-3608.

Please bring any records from your previous doctor, a list of your medications, allergies and a list of prior doctors and other previous health care providers and practitioners. We also recommend bringing in a list of any questions or concerns you may have. Please make sure that you arrive at least 15 minutes before your scheduled appointment so that you can provide us the necessary paperwork and insurance information to put you in our system. To save time, you can also print the Intake Forms located on our forms page, and bring them completed to your appointment.

We highly recommend you bring comfortable, nonrestrictive clothing or athletic wear to our physical rehab clinic because oftentimes we will be stretching or having you exercise. Your clothes should also allow you to move freely so please dress appropriately. If you have any questions, please contact us prior to your first appointment.

Again that depends on the same factors associated with how long treatment will take. We make a treatment plan for you, which includes follow up and regular maintenance visits to our physical rehab clinic. If at any point you feel like you are not progressing at a satisfactory rate, we will collaborate with you and modify your treatment plan. We will always keep you informed as well about your progress. Many factors enter into the overall picture of any one plan of care. We are simply focused on the results.

Generally, most insurance companies pay for a percentage of the physical therapy visit. It depends on your chosen coverage plan. What you need to know, clearly and before you enter our system of care at Body Technic Systems, Inc. is we are not part of any insurance “panel.” We are classified as an “out of network provider.”

We recommend that you call the benefit number listed on your insurance card to check on your insurance benefits before you start therapy. You can request we call the benefit # on your insurance card. There is no charge for this service. You need to provide a copy of the front and back of your insurance card to us.

Most patients save time and money in the long run because our approach is centered on treating the underlying problem. With over 15 years of treating and assisting patients we have found a reduction in visits by our patients by over 60%.

Many patients have us submit their paperwork to our billing company to assist in helping with reimbursement. Reimbursement varies depending on your insurance provider and the plan you have chosen. There is no extra fee for this service.

After your initial evaluation we will send a copy of the examination to your doctor along with progress notes at regular intervals, as is necessary/and/ or as requested.
